Privacy Notice

• Hampshire County Council (Public Health Service – HCC) and Isle of Wight Council ( Public Health Service – IWC) are required by the Local Authorities (Public Health Functions and Entry to Premises by Local Healthwatch representatives) Regulations 2013, to offer an NHS Health Check to their eligible population once every five years. They have commissioned a third-party provider Randox Health Ltd to carry out this service.  HCC will be responsible for the personal data relating to patients in Hampshire as a data controller and IWC will be responsible for the personal data of patients on the Isle of Wight as a data controller, but neither will have access to the health check data.
• The information provided in your booking will be used by our provider Randox Health

(for Hampshire) and their sub-contractor Lighthouse Medical Ltd (for Isle of Wight), to process your health check booking.  When you attend an appointment, Randox Health or Lighthouse will carry out a health check and record your personal data relating to that check on the health check system, which is provided by Health Diagnostics Ltd.  The personal data will also be shared with your GP who will be responsible for maintaining that data as part of the patient record.  Your GP will share pseudonymised and aggregated data with HCC or IWC for the purposes of performance monitoring for the service.  If you do not want your data to be pseudonymised and shared with HCC by your GP, please tell your GP practice. If you refuse sharing of your data by your GP, you can still access the service.

• The legal basis for our use of this information is that processing of the personal data is necessary for the performance of a task which is in the public interest and for reasons of public interest regarding public health.

• Your data will be stored in the UK.
• We will keep your personal information for 7 years
• You have some legal rights in respect of the personal information we collect from you.
